T-mobile sells sim and 125 minutes prepaid activation kits($50) in store
but you probably won't get gsm in rural areas with any proviider(at
least not in nevada)

We have used Cingular prepaid in California and Nevada just fine, SIM
card plus x minutes (don't remember) was 50 bucks.
